We hope that the care you received from my staff and myself will encourage you to send your friend, neighbor and coworker to us for their dental care. For that reason we have developed a rewards program just for you.
With this card you can give a $50.00 discount to apply toward their 1st visit. As a result, when they redeem their credit, you will receive a $50.00 dental care discount for your next visit. There is no limit to the number of discounts you can receive. This discount can be put toward routine dental care, cosmetic dental service, or our Opalescence Whitening System.

- Your discount is not redeemable for cash or toward previously completed dental treatment.
- You will receive one discount per referral, and may apply this amount toward treatment for yourself, your spouse, your child, or other member of your household.
- You will be rewarded for each new family you refer, not for each member of that family.
